Jamie Coburn Van Horn is an Occupational Therapist who specializes in integrative health and prevention. She graduated from The Ohio State University with a B.S. in Allied Health/Occupational Therapy.
She is the Founder and practicing therapist of Align Health, LLC. She has a deep understanding of the mind-body connection having provided care as an OT over 20+ years within corporate consulting, outpatient orthopedics, industrial rehabilitation, pediatrics, private practice and home health.
She is trained in lifestyle management, functional movement, augmented soft tissue mobilization, postural correction, dementia management, mindfulness, parent coaching, sensory integration, ADHD, Executive functioning, pediatrics, workplace ergonomics/functional capacity evaluation and home modifications. Jamie focuses on aligning and serving the whole person – mind and body, including the environment.
Dr. Kristen Fogle is a physical therapist specializing in the treatment of orthopedic and women’s health related conditions. She attended Mercyhurst College in Erie, PA, where she received her Bachelors degree in Sports Medicine with a concentration in Physical Therapy in 2010. From there, she went to the University of Toledo, graduating with her Doctorate degree in Physical Therapy in 2013. Upon receiving her Doctorate degree, Dr. Fogle came home to Columbus, Ohio, where she has been working as a physical therapist in the outpatient settings.
Currently, Dr. Fogle has been focused on working with clients in the outpatient setting dealing with women's health related conditions, orthopedic related injuries, post-operative conditions and achieving overall wellness through personal training. She completed her women's health certification with Herman & Wallace to be able to provide exceptional care for women dealing with different conditions of pelvic floor pain, pregnancy, and post-partum recovery.
Dr. Fogle is passionate about fitness, being active by running and in Cross Fit. She brings this passion and the techniques utilized in these workouts to her approach with clients. She places a high priority on patient education, enabling clients to take ownership of their recovery and maintain optimal function and fitness levels outside of the clinic.
Max Shultz is a Licensed Massage Therapist (LMT). He earned his Associates degree from Hocking College in Massage Therapy in 2012. Max chose to further his education to become a Licensed Physical Therapy Assistant (PTA), completing his Associates degree, again, at Hocking College in 2019. He is currently using both degrees in a professional capacity.
As a LMT with eleven years experience, Max focuses on employing therapies to address stress and/or pain management and the natural aging process on the body. He addresses these by using Swedish relaxation massage, deep-tissue and trigger-point therapy approaches.
By using a variety of techniques to cater to each individual's needs, Max is dedicated to moving clients toward physically embracing living in a positive way. While Max is schooled in a variety of techniques, he enjoys learning and continuing to develop his skills to better help his clients.

Nicole's mission is to serve others on their journey of health and lifestyle management to promote one's quality of life and ability to function optimally in daily living. Nicole is a nationally certified and state licensed occupational therapist, with an Occupational Therapy Doctorate from The Ohio State University. Nicole obtained a Bachelor of Science in Kinesiology and Nutrition Minor from Miami University. Nicole is a Certified Personal Trainer (CPT) from the National Academy of Sports Medicine and has advanced certificates in health management inclusive of University of Southern California certificates in lifestyle management for mental health and for weight management, diabetes, and related co-morbid conditions. As a soon-to-be Certified Wellness Practitioner through the National Wellness Institute, Nicole embodies a passion for continual knowledge advancement and evidence-based services. With her knowledge and passion, Nicole empowers others in optimal health and wellness by fostering sustainable, satisfying, meaningful, and health-promoting habits, routines, and behaviors. Nicole does so through provision of individual, group, and community services inclusive of community education seminars and initiatives, with emphasis on a client-centered, comprehensive, and proactive approach to assist in navigating barriers to enhanced health outcomes, well-being, and quality of living. Pauline is native to Columbus OH and was a Buckeye for her undergraduate studies. She received her Bachelors degree in Human Development and Family Science. Pauline went on to receive her Doctorate of Occupational Therapy from Jacksonville University. Her clinical studies included pediatrics and feeding, adult neurological conditions, and adaptive sports and recreation.
Since then, she has developed her skills in sports performance & neurodiverse pediatrics to enhance the personalized experience and improve quality of life.
With her free time, Pauline is a competitive gymnastics coach. Her gymnastics passion has led her towards developing an injury prevention and sports wellness training to help her athletes continue to perform their best mentally, emotionally, and physically.
