Please note that Align Health is an “Out of Network” (OON) provider and does not contract with any insurance companies and, in accordance with federal regulations, is unable to serve Medicare B recipients for Occupational/Physical Therapy services.
This means payment for services is collected at the time of service and that our clients are responsible for submitting a claim to insurance providers. Upon request, clients are provided with a detailed superbill that may be used for self-submission of claims.
This model allows us to provide personalized, one-on-one care with the same OT/PT throughout the treatment plan. It is our belief, clients are able to achieve desired outcomes faster and with less out-of-pocket expense. Align Health OT/PT's conduct treatment through agreed upon sessions between client-therapist, not insurance protocols/restrictions. When insurance dictates the course of treatment, there is often a reduction in personalized care and patient outcomes. We have discovered our model of delivering care aligns the client with his/her goals in a shorter duration of time.

As a small clinic cancellations without optimal notice can be impactful to the Align Health professional. As a result a $50 cancellation fee will be applied to appointments cancelled with less than 24 business hours notice and, after a second late cancellation, the client will be charged the full rate of the appointment.
