Align Health Social Worker's are experienced within the areas of: 

• Assisting individuals with personal factors impacting health/wellness
• Aging-in-place coordination 
• Connection for school, home, and community services to help children with emotional, developmental, and educational needs. 
• Facilitate support services for children, older adults & people living with chronic conditions
• Advocacy
• Aligning services for older adults to promote optimal living with dignity. Focusing on the biopsychosocial & economic aspects of daily living. 
• Child welfare - specializing in strength-based approach of child/family to promote highest functional independence 
• Assisting parents of children with development disabilities through advocacy

Services Include: Multi-dimensional assessment comprised of evaluating the needs of an individual and/or extended support system, developing a plan aligned with resources to achieve optimal well-being. 

A Social Work assessment incorporates different aspects specific to the need - environmental, cultural, social, and personal variables. The primary goal is to reach one’s full potential and provide support needed through the situation.
