Jamie Coburn Van Horn is an Occupational Therapist who specializes in integrative health and prevention. She graduated from The Ohio State University with a B.S. in Allied Health/Occupational Therapy.
She is the Founder and practicing therapist of Align Health, LLC. She has a deep understanding of the mind-body connection having provided care as an OT over 20+ years within corporate consulting, outpatient orthopedics, industrial rehabilitation, pediatrics, private practice and home health.
She is trained in lifestyle management, functional movement, augmented soft tissue mobilization, postural correction, dementia management, mindfulness, workplace ergonomics/functional capacity evaluation and home modifications. Jamie focuses on aligning and serving the whole person – mind and body, including the environment.
Dr. Kristen Fogle is a physical therapist specializing in the treatment of orthopedic and women’s health related conditions. She attended Mercyhurst College in Erie, PA, where she received her Bachelors degree in Sports Medicine with a concentration in Physical Therapy in 2010. From there, she went to the University of Toledo, graduating with her Doctorate degree in Physical Therapy in 2013. Upon receiving her Doctorate degree, Dr. Fogle came home to Columbus, Ohio, where she has been working as a physical therapist in the inpatient and outpatient settings.
Currently, Dr. Fogle has been focused on working with clients in the outpatient setting dealing with women's health related conditions, orthopedic related injuries, and post-operative conditions. She completed her women's health certification with Herman & Wallace to be able to provide exceptional care for women dealing with different conditions of pelvic floor pain, pregnancy, and post-partum recovery.
Dr. Fogle is passionate about fitness, being active by running and in Crossfit. She brings this passion and the techniques utilized in these workouts to her treatment style. She places a high priority on patient education, enabling clients to take ownership of their recovery and maintain optimal function and fitness levels outside of the clinic.
Carrie Jarrett, LMT has been licensed with The State of Ohio Medical Board for massage therapy since 2003 when she graduated from AIAM – American Institute of Alternative Medicine. She is certified in kinesiology taping and Medical Massage.
Carrie has worked with area healthcare providers treating such issues as: TMJ, sports injuries, car accidents (whiplash), migraines, eating disorders, abuse, scoliosis, certain forms of muscular dystrophy, paraplegics/ quadriplegics, effects of aging, prenatal care/self-help and many more.
In working within the community, she’s a member of Heidi’s Fatigue Fighters who provide care to visiting performers, athletes and entertainers. Being on the road often means very little health care or self-care maintenance. Working with local physical/occupational therapists, reflexologists and chiropractors, helps these client's achieve optimal performance when on the road.
Carrie continues to collaborate with medical professionals in delivering massage therapy to assist in a client's choice of care.
Karen is a Certified Life Coach and a Channel Partner with The Ken Blanchard Companies. In her Life Coaching practice, she is focused on helping people navigate through life’s transitions and find their fulfillment and happiness again. She understands the struggles first hand and guides clients with non-judgment, empathy and accessible tactics and tools.
She has a unique understanding that transitions can often lead to losing yourself and your zest for life. She utilizes coaching tactics and mindset exercises to help guide clients from letting life happen to them to intentionally directing their life with excitement and enthusiasm.

Jessica Lemmons is a licensed occupational therapy assistant who graduated from Zane State College. She has been practicing for 8+ years with a focus in the pediatric population. She has experience with serving clients from birth through adulthood with various levels of abilities and diagnoses. Through her years of providing occupational therapy treatments, she has gained skills in a variety of settings including outpatient clinics, hippotherapy, home health, schools, and for the past three years, telehealth.
Jessica enjoys assisting children in improving the many skills required to be as independent as possible at home, school, and in the community. Some of these skills include self-regulation, fine motor and visual motor skills, handwriting, executive functioning, sensory regulation, and self-care skills. To be able to provide a fun, functional, and productive treatment, Jessica does her best to incorporate the clients personal interests and integrates preferred play and enjoyable activities in to her sessions whether they are completed face-to-face or virtually.

Ever since A'Donna Fuller was 10 years old, she has always loved cooking! Throughout the years, she has enjoyed cooking for individuals, families and children. A'Donna has aligned passion with formal training from The Art Institute of Michigan for Culinary Management to deliver nutritional coaching to those seeking optimal lifestyle management.
As a personal chef, A'Donna has made it possible for people to come together through food. Her vision is a world where culinary medicine is key in the management of illness. She has been afforded the opportunity to volunteer for a host of hunger relief and food education agencies like, Cooking Matters.
It was, however, through her own journey of chronic pain, that she discovered the healing properties of food. She has applied her acquired knowledge to instruct and support others navigating this journey. It is her hope to combine cooking and food education for the empowerment of holistic healing.
Juli is a Reiki Master Practitioner in the Usui Reiki Ryoho lineage. She began her journey to reiki shortly after she completed her RYT200 yoga instructor training at Yoga on High in 2015. As someone who has long suffered with anxiety and overwhelm, she was so drawn to the detectable calm, clarity and restoration felt after each Reiki session with her mentors and knew that this type of energy healing that she wanted to offer to her students. She finished her Reiki Level 1 and 2 certifications and most recently, her Level 3 (Reiki Master) in 2020.

"Here is what I think when I remember my first time meeting you.
Your confidence from your years of experience instantly drew me in and made me feel comfortable. There was no beating around the bush. Before all of your assessments were even done you validated my pain and were able to predict what you thought was causing it all - I breathed a sigh of relief because IMMEDIATELY you said you weren't going to waste my time (or yours) if you felt like you couldn't help after a certain amount of visits.
All of the other therapists told me so many things they wanted to "try" on me, got baffled when it didn't work and were quick to dismiss me and my pain. From my viewpoint you were digging deep to uncover things other therapists didn't talk about - I'm sure from your viewpoint it was just the basics. These were things I was doing daily that I didn't know were working against me. You started educating me about the importance of better food choices, posture while driving/working, increasing hydration, epsom salt/baking soda/lavender soaks, reducing alcohol and stress, keeping a food/activity journal to watch for pain triggers, and varied cardio to track and manage my pain.
You were patient and educated me about my body through each part of the process and about every maneuver or tool you used on me as well as what to expect afterwards. You didn't just have me going through the motions. You coached me through breakdowns without the pity parties even when I thought there was no end in sight. I went from sitting in my car crying after appointments with other impatient and less knowledgeable therapists (because once they came to an impasse with my care, they flipped a switch and turned mean and dismissive as though I was making up or exaggerating my pain) to looking forward to coming to therapy with you. My entire family knew my therapy was changing for the better because they said I sounded excited and less defeated when I talked about "my new therapist"
You realized how this was impacting my quality of life and empowered me with tips and tools to manage even before I started feeling better. It was through those tools I started taking time for myself outside of therapy, including setting emotional and physical boundaries with others and myself to make sure nothing got in the way of my new routine and goals I was working toward.
But overall your no nonsense approach, warm and upbeat personality is what would make me refer everyone to you I know - not to mention, you know your stuff!"

After a few months of reducing my training volume and taking more rest than I wanted too, I reached out to Kristen to see if she could help with some lower body issues that were only getting worse and seemed to compound on each other. Chef A’Donna provides a no judgement learning environment and meets you right where you are in life. She’s extremely passionate about the health and wellness properties food can provide offering a fresh perspective on how to utilize everyday ingredients in new ways. This is great for a mom like me because I struggle trying to come up with new ideas to incorporate more veggies into my kids meals. Her classes are extremely educational and everything that my family has tried so far from meatless meatballs to veggie sliders, has been a success. It’s the little victories that count.

Kristen did an incredible job of assessing my needs and understanding my goals so she could put together the right plan for my success. As a long distance runner it was important I was able to get back to running safely after having my baby. With each exercise Kristen explained what aspect of my recovery it was targeting and how it was engaging my overall system. In addition, she explained how that helped in engaging with my baby, making me mentally aware of my posture both during and out of our sessions. She was able to translate and demonstrate the exercises easily through video and having the telehealth option was critical to my ability to stick with our appointments and my therapy plan. It is clear Kristen is passionate about helping postpartum women in their recovery, and I would highly recommend working with her!
My daughter, Lexi, is 15 and has been in early intervention therapy since she was the young age of 3 months old. She has had numerous therapists over the years however, Jessica stands apart from the rest. As a parent, it is comforting to have an Occupational Therapy Assistant that is so compassionate and knowledgeable. It's even better, when your child senses that compassion and gives their best to whatever fun task is ahead. Jessica is so creative and adapts any given project to suit my daughter's needs, all while helping her improve her capabilities and striving to reach her goals. Jessica is AMAZING at what she does! I am grateful for the difference she makes in my daughter's life!
